Physical Exams

Physical Exams:

 

The Pennsylvania School Health Act requires a physical examination for every student in Kindergarten, sixth, and eleventh grade. 

It is recommended that this examination be completed by your family physician since he/she can best evaluate your child's health needs and assist with any necessary treatments, corrections, and immunizations.  A copy of your child's junior learner's permit is also an acceptable form of proof of physical exam.  Please return all completed physical forms to the nurse's office as soon as possible.  You may fax or have your child's physician's office fax the physical form to the nurse. The fax number is 717-762-3787.  

Please note **You will not be permitted to participate in "Senior Privilege" activities your senior year if a completed school or sports physical is not on file.**

Attention Sophomore's - Your Junior physical can be completed anytime during your Sophomore or Junior year.  If you have an appointment set up with your doctor please be sure to take the appropriate forms with you.
