PA Immunization Requirements:

For attendance in ALL grades students will need the following:

4 doses of tetanus (one dose on or after the 4th birthday)
4 doses of diptheria (one dose on or after the 4th birthday)
3 doses of Polio
2 doses of measles
2 doses of mumps
1 dose of rubella
3 doses of hepatitis B
2 doses of varicella (chickenpox) vaccine or history of disease

Proof of immunizations must be in writing. A signed physician's note stating the date that the vaccine was given or a copy of your child's vaccination record with the dates the vaccines were administered are acceptable proof. Your child is considered a provisional student and must provide proof to the school nurse by November 1st to be able to participate in school or any school related functions until the requirements are met. If your child is unable to attend due to immunization violations these days will be considered unlawful. 

*If you need financial assistance with obtaining these vaccines, please contact the Franklin County State Health Center at 717-263-4143 as soon as possible.  They will only treat patients that do not have health care insurance.  For further assistance or questions please contact me.
